You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. Alternatively, you can access the form here.

If you have already self certified and require a further sick note, please contact the surgery to arrange a GP appointment.
If you require a continuation of a long term sick note, please email pearlmedicalcentre1@nhs.net with your name, date of birth, address and reason for the sick note.
